Pregnancy Massage Intake Form
For any pregnant clients, this form is needed before receiving Prenatal massage.

Medical Massage and Neuromuscular Therapy Intake Form
For your 1st Medical Massage or Neuromuscular Therapy session, which includes an assessment, this form is required. We highly recommend you complete it and email it to massageconnections@gmail.com BEFORE the day of your appointment to allow your therapist time to review the information.

Manual Lymphatic Drainage Intake Form
Manual Lymphatic Drainage is not a massage session and the reasons for receiving it are unique to that type of session. Whether you are looking for post-surgical healing or immune-boosting benefits, this form is needed in your first session so that your therapist can plan the most appropriate procedures for success.

Functional Integration Client Intake Form
The first session always includes a full body assessment and plan for the client. Each session is different depending on the body’s need. Like finding the right combination to unlock a lock, Functional Integration accesses, activates and reboots the body’s soft tissue, movement patterns, and the brain and nervous system with healthy new patterns.
